Insurance & Enrollment

To begin therapy  we will need:

1. Doctors Prescription for therapy services (OT and/or ST) with ICD-9 code and
   frequency of visits
2. Completed New Patient Welcome Packet along with signed Financial Policy/Assignment of Benefits: Please complete and fax to: 770-886-8617 24 hrs prior to scheduled service date so we can call in for benefits verification.  Failure to complete prior to services will require you to pay in full at time of appointment.
3. Current evaluation (less than 12 months old)-Parallel Play can provide an occupational and/or speech therapy evaluation if one has not been completed in the last 12 months
5. Insurance Information: Copy the Front & Back of your primary and secondary insurance
6. Medicaid Information: A copy of Medicaid card and a copy of the current months eligibility form

Fees: Parallel Play accepts private pay, insurance and Medicaid.  We are an out of network insurance provider for all carriers, except Blue Cross Blue Shield PPO and POS. Patients may have different in and out of network benefit coverage. Please complete and fax Intake form and Financial Policy to: 770-886-8617 prior to scheduled service date so we can call in for benefits verification.

All co-insurance, co-payments and payments to deductible are due at the time of service.
